Seventeen patients underwent pancreatic EUS-FNA for the first time. Twenty-seven patients had undergone prior transabdominal ultrasound (TAS)-guided FNA. Specimens were interpreted as satisfactory or unsatisfactory for evaluation by cytologic and histopathologic criteria. Results of EUS-FNA were correlated with the results of TAS-guided FNA, final diagnosis, and clinical outcome in cases with malignant pancreatic lesions. Diagnostic accuracy was 75.5% (34 of 44 procedures) and sensitivity was 88.5% (22 of 25). Adequate specimens were obtained in 86.7% (19 of 22) of cases with malignant lesions and in 63.6% (seven of 11) of cases with benign lesions. There was one false-negative cytologic result and four patients (11.1%) were found to have a definitive diagnosis of malignancy on resection. There was no morbidity or mortality associated with EUS-FNA.
